FMCSA Drug & Alcohol Clearinghouse

I provide written consent to Sonic Transport Inc. (USDOT No. 2047019) to conduct a pre-employment full query of the FMCSA Drug & Alcohol Clearinghouse, as required by 49 CFR § 382.701. I understand a full query will return information about any drug and alcohol program violations. Results may be used in determining my eligibility. I have the right to review my Clearinghouse record at clearinghouse.fmcsa.dot.gov.

Pre-Employment DOT Drug Test

Federal law (49 CFR § 382.301) requires a pre-employment DOT drug test before operating any CMV. The test screens for marijuana, cocaine, opiates, amphetamines, and PCP. A positive result, adulterated specimen, or refusal to test will disqualify me from employment and will be reported to the FMCSA Clearinghouse. I have not tested positive or refused a DOT drug test in the past two years without completing return-to-duty.

Disclosure and Authorization for Consumer Report

This disclosure consists solely of the following, as required by the Fair Credit Reporting Act (15 U.S.C. § 1681 et seq.)

Sonic Transport Inc. may obtain a consumer report and/or investigative consumer report about you from one or more consumer reporting agencies. These reports may include criminal history, driving record (MVR), employment history, and other information. You have the right to request a written description of the scope of any investigative consumer report and a copy of your report. A summary of your rights under the FCRA is available at consumerfinance.gov/learnmore.

I authorize Sonic Transport Inc. to obtain my motor vehicle record(s) from any state DMV for employment purposes, as permitted under the Driver's Privacy Protection Act (18 U.S.C. § 2721(b)(3)).
